special services for you and your child under this Medicaid-covered program.

PHRM Services
If you're eligible and decide to take part in the PHRM program, our team of nurses, nutritionists and social workers
will help you with:
- Finding a doctor for your pregnancy and for your baby
- Medical referrals
- Delivery planning
- Health information and diet advice
- Family planning
- Home visits

- Learn about your pregnancy
