Planning for all our health programs is based on the careful analysis of needs and the establishment of specific goals and measurable outcomes. Using decision science methodology, our planning tools allow us to identify the areas and populations of the state most at need, and then to choose the most effective way of meeting those needs with the funding and resources available.

We use a set of carefully selected and weighted health indicators to evaluate the current health status of each county, and interactive tools such as our county planning module to project improvements possible by new health programs.

Health programs that we implement are guided and measured by logic models that clarify the structure of the program's funding, participants, goals and performance.
