How can I apply for an Individual Onsite Wastewater System?
You can download the Application Form from this website. It can be completed and returned with the required documentation in person or by mail to the county health department in which the property lies.
Can I complete the process online?
Not at this time. While the state can process credit cards online, our process requires additional information which is not electronic.
How much does the application cost?
The fee for an inspection or a soil site evaluation is $50. Health departments will accept cash, check or money orders. Credit cards are not accepted at this time.
How long does it take?
The workload in each Health Department varies with the time of year and the amount of construction being done in that county. We are required to do an inspection within five working days and provide a recommendation to you ten days after that.
Why might it take longer?
If there are problems with your lot or existing system, our environmentalists may seek additional information or advice from our experts. This may take longer, but you should receive a status report to this effect.
What additional documentation is needed?
- 911 Address - A 911 address must be provided before the application can be processed. This is available from the county 911 coordinator.
- Legal Description - You must provide us with a copy of the deed to the property, or a legal description. We will look for Range, Township and Section information. This enables us to determine the size of your lot and its ownership. Each dwelling on a property must be either on a separately surveyed lot or have an easement for use of enough land for the system.
- Plot Plan - This is a diagram of the layout of the house, water well, outbuildings, driveways and wastewater system.
- Homeowner Affidavit - A legally bind description of the wastewater system given by the homeowner. This is required for an existing system, but not new construction.
- Treatment Plant Inspection - A certification made by a manufacturer's representative that the treatment plant is operating correctly. This is required for existing treatment plants but not existing septic tanks.
- Easement - if you do not own the land on which the dwelling is to be built, or if you do not own enough land, then an easement acquiring enough land will be required. There is a model easement form on this website.
- Disclaimer - Only used for water connections to property on which no house is to be built.
